Position Summary

Responsible for the effective and efficient scheduling of clinician visits to patient homes while supporting optimal patient care, clinician productivity, and operational excellence. This role serves as a central point of coordination between patients, clinicians, and agency leadership to ensure visit schedules are accurate, timely, and responsive to patient needs.

Responsibilities

  • Utilize Homecare Homebase (HCHB) to create and maintain clinician schedules
  • Coordinate patient visits based on clinician availability, geography, and patient needs
  • Communicate scheduling updates and changes to clinicians, patients, and leadership
  • Monitor schedules to support productivity, visit utilization, and timely patient care
  • Provide administrative support including word processing, correspondence, filing, and document management
  • Answer incoming calls and respond professionally to inquiries
  • Take accurate and concise messages and route communications appropriately
  • Compile information and prepare reports using agency systems and records
  • Maintain confidentiality and compliance with agency policies and healthcare regulations
  • Collaborate with clinical and operational teams to support daily workflow needs

Qualifications

  • Minimum of one (1) year of administrative experience in a professional office environment preferred
  • Previous home health scheduling experience strongly preferred
  • Experience with Homecare Homebase (HCHB PointCare) preferred
  • Strong organizational and time-management skills
  • Excellent communication and customer service abilities
  • Ability to manage multiple priorities in a fast-paced environment
  • Proficiency with Microsoft Office and computer-based scheduling systems
  • Ability to work collaboratively within a team-oriented culture
